Summary:"The United States is on the brink of a permanent shift in the population of people over the age of 65. Due to advances in medical care that have allowed life expectancy to increase, people are now living longer, but with chronic conditions that are more likely to require costly long-term care. Although the medical model continues to dominate care of the elderly, a growing budget crisis related to the costs of long-term care has encouraged policy makers and experts to consider alternative approaches and strategies for elder care. Cost savings may be realized by delaying entry into more expensive skilled nursing facilities and allowing the elderly to remain in the community. A paradigm shift toward a focus on a holistic model of care management, which links service delivery systems and attends to the prevention of chronic conditions, has gained some traction in social work practice. The Initiative is one example of an innovative approach to care management. It seeks to build on individuals' strengths and interests, facilitate community involvement, provide opportunities for self-care and health promotion activities, and delay more costly expenditures in formal long-term care facilities. According to feedback from consumers and key informants involved with The Initiative, the initial results are promising."--leaf 3.
